The difference in reaction is caused by the pliability of the skin. The smaller the cylinder, the less skin surface area the cylinder occupies, the less pliable and harder it is to pull the penis away from the body. The larger the cylinder, ie the penis & scrotum cylinder, allows for the least skin resistance and the most flexibility for your penis to be pulled away from the body resulting in more pull on the suspensory ligament which equals more length.

As of this writting, Dr Joel Kaplan does not incorporate rubberized seals at the base of his cylinders as does all PumpToy products. Having a seal at the base makes it much more comfortable to pump as the hard plastic does not dig into the body. A good seal is also easier to obtain with a rubberized base.
